FPO Checklist

Reminders:

  1. See Melissa Lawson as soon as you have a target FPO date in mind.
  2. Date on title page is month/year of graduation, not FPO date.
  3. Remember to acknowledge your funding in your thesis. See advisor for official grant numbers.
  4. Read the formatting and binding guidelines carefully. See this page for formal requirements.
  5. Forms must be filled out on line (PDF) and printed out, not handwritten.

Two Weeks Before Requested FPO Date:

    You give me:

  1. Two Reader's Reports (Click here for the form--MUST BE TYPED)
  2. Advisor's Report (Click here for the form--MUST BE TYPED)
  3. Degree Application (See Melissa Lawson)
  4. Thesis copies:
    • One unbound copy for display.
    • Two copies of abstract, title page, and copyright page.

    You also:

  5. Confirm date and time of FPO with advisor and two non-readers, who must attend.

    I will:

  6. Submit Request to Hold FPO along with your paperwork.

Additional Paperwork:

    Day of FPO, following the FPO:

  1. Three bound copies of thesis with last name, year on spine. These three must be single sided, and can be single spaced. You may choose to make additional copies for yourself, your advisor, readers, etc. These can be double sided.
  2. Check or Money Order payable to Princeton University Library for $135 (degree + copyright fee) or $80 (degree only).
  3. UMI Dissertation form (option TR-1 only).
  4. One copy of double spaced thesis in PDF format, as a single file, on a CD-rom, in jewel case; CD and case labeled with Princeton University, Author last name, Author first name, Thesis title.
  5. Survey of Earned Doctorates. (Click here for survey) Register and complete online. When finished print out Certificate of Completion page and give to Melissa.
  6. Exit Questionnaire. (Click here for form 1)
  7. Termination form (this can be done in advance).
  8. Send PDF final version of thesis to Tech Reports Librarian (mml@cs) along with a plain-text version of the title and abstract.

    I will:

  1. Submit signed FPO Exam Result form and bound copies to Mudd Library, and submit final paperwork to Grad School.
